All patients have the responsibility to:

• Upload authentic your identification documents
• Provide complete and accurate information about current condition, current treatments and medical history
• Ensure they understand their treatment plan and instructions by verbally checking
• Follow instructions of treating plan as advised by the physician
• To attend all appointments on time and inform the clinic in the case of time change or cancellation
